Transition
The transition from adolescence to adulthood is a process that all young people experience, but for students with disabilities, the process can take on added importance and complexity.
Whenever a student receiving special education services from SSD reaches age 16, the district’s staff – in collaboration with the student and his or her family – develop a transition plan as part of the IEP process.
Get a step-by-step look at the transition process.
