Homebound Application

The application is a four-part PDF document that can be downloaded and saved to your computer, filled out and submitted electronically. Repeating fields on Parts 3 and 4 will be auto-filled if you complete the document on your computer.

Homebound Information
  • SSD's Homebound Department Mission:
    To collaborate with partner districts' general and special education staff and parents to provide and support highly qualified teachers (for students who are unable to access services in a school setting) by providing a system to gather data, provide training, offer support and efficiently assign, deliver and report services in a professional and positive manner.
